An information sheet is __ / is not ___ enclosed for distribution (please limit to one side letter size bio/credentials).  I (Print Name) _____________________________, certify that I have read the Association's declaration of condominium, articles of incorporation, bylaws, and current written policies and will work to uphold such documents and policies to the best of my ability and that I will
faithfully discharge my fiduciary responsibility to the association's members.

BELOW PLEAS FIND ELIGIBILITY REQUIREMENTS TO SERVE ON THE BOARD OF DIRECTORS:
1. In a condominium with more than 10 units, co-owners of the unit cannot serve on the Board at the same time, unless the co-owners own more than one unit.
2. Directors suspended or removed by the Division are not eligible to serve during the time period the suspension or removal is pending;
3. If you are more than 90 days delinquent in the payment of any monetary obligation you may not serve on the Board of Directors;
4. If you have bee~ convicted of a felony in Florida or convicted of an offense in another jurisdiction that would be considered a felony in Florida (unless your civil rights have been restored for a period of no less than five years as of the date of the election) you may not serve on the Board of Directors; and/or 5. If you are currently charged by information or indictment with a felony theft or embezzlement offense involving the Association's funds or property and such criminal charge is pending you may not serve on the Board of Directors.
